If you have completed all steps under Seaching for a Permit and you are certain that the permit is missing from the database:

  1. Under the Heading "Submit", click on  "Missing Permit".
  2. Fill out the form that appears with as much information as possible.
  3. Make sure to Add a Component (the type of system that is onsite).
  4. The system may require you to fill in information if it was not filled in previously.
  5. When finished, scroll to the top of the page and click on "Save Permit."
  6. The new permit form will appear as blank but this actually indicates a successful save.
  7. A system administrator will review your permit. If there are any issues, we will contact you.
  8. You should be able to add inspections and/or sample results immediately. See Inspection Submission or Sample Submission for instruction.
